1. Click on the “PICK YOUR TEAM AND APPLY” box.
2. Pop-up appears saying “Pick your favorite team”
3. Under “CONFIRM CARD ART” box click on the Chicago Bears Logo(a Bears head).
4. Once the Bears Head is highlighted, click the “CONFIRM CARD ART” box.
5. The application form appears.
6. Fill out the information.
7. Once completed click on the APPLY button at bottom of page.
